VMAP Project ECHO Early Childhood SOCKs: Systems of Care for Kids takes an “all teach, all learn” approach to understanding when, how, and where to refer families of young children. Community-based subject matter experts will be paired with an interdisciplinary faculty team to provide practical and actionable infant and toddler connections for supportive pediatric primary care.

  • Earn up to 75 CE/CME points. Cohort members may also participate in an optional MOC-4 QI Project to help enhance screening and referral for social determinants of health supports.

    CME/MOC package offered at a reduced rate of $50.
